Relationship Between Prognosis and Length of Serosal Invasion in Gastric Carcinoma


Abstract
166 cases of advanced gastric carcinoma invading the serosa were studied with regard to the relationship between the length of the serosal invasion(L.S.I.) and prognosis with a division made between expanded and infiltrative types of tumor during
the 40
months from September, 1991 to December, 1994 in the Department of General Surgery. Among patients with expanded type of positive serosal invasion, average 3 year survival rates were 72% and not related with L.S.I. certainly. Among those patients
with
infiltrative type of positive serosal invasion, overall 3 year suvival rates(66%) were lower than that of ezpanded type. The 3 year survival rates were worsened in proportion to increment of L.S.I. and it was 28% in 4 to 8cm in L.S.I. and 33% in
over
12cm in L.S.I. In expanded type, liver metastases were more frequent than in infiltrative type and increased in proportion to increment of L.S.I. Peritioeal infiltration and direct invasion to neighboring organs were more frequent in infiltrative
type
than in expanded type and were increased in proportion to increment of L.S.I. Statistical analysis of survival rate showed significant difference in association with or without serosal invasion(P=0.0041) and showed significant difference in
association
with or without serosal invasion(P=0.0041) and histological types in cases of serosal invasion(P=0.0254). Knowledge of these factors facilitates the planning of operative methods and operative adjuvant treatment.
